Payment

Payment is required at the beginning of each session. 

We accept all major credit cards, checks and cash, as well as HSA and FSA cards.

Fees

Standard Rate: $175.00,  Self Pay Discount Rate $150.00

Phone Consultation: $50 - $150

Document: $50 - $150

Court Appearance: $250 - $750

Cancellation Policy

A minimum of 24 hours notice is required for canceling or rescheduling an appointment.  Appointments are made to reserve a specific time for you. The full fee will be charged for appointments that are missed, canceled or rescheduled without 24 hours notice.

As much as insurance policies differ regarding coverage and limitations, you are strongly encouraged to contact your insurance carrier to determine the limits of your coverage. Please remember that any charge not covered by your insurance will be your responsibility. Your portion of the payment is required to be paid at the time of service.
